Consent to Receive the Flu/Covid Vaccination
Flu vaccinations at our practice may be administered by either a nurse or a suitably trained Healthcare Assistant (HCA).
Before booking or if you have already booked your flu vaccination appointment, then please ensure that you have read the following information:

By booking a flu/covid vaccination appointment, you are confirming that:
- You have read and understood the information provided about the flu vaccine.
- You have had the opportunity to consider whether you wish to receive the vaccination.
- You consent to receiving the flu vaccination at your appointment.
You will also receive a text message asking you to confirm your consent to receive the flu vaccination. Please reply to this message before your appointment. If your consent is not confirmed in good time, this may delay your vaccination
If you have any questions about the flu vaccine, would like further information, or are unsure whether the vaccination is suitable for you please contact the surgery to arrange an appointment with a GP or Practice Nurse. They will be happy to discuss the vaccination with you and answer any questions before you make a decision.
On the day of your appointment
The clinician administering the vaccine will:
- Confirm your identity.
- Check that you are eligible for the vaccination.
- Confirm that you are still happy to receive the vaccine.
