Registration
When you register you will be asked to complete the following forms:
- Registration form
- Medical questionnaire, it may take considerable time for us to receive your medical records
- Alcohol Screening
Once forms are completed you need to book an appointment with our Health Care Assistant for a Healthcheck
GP2GP is the technology that transfers your electronic health record directly and securely from your previous GP when you register at this practice.
Online 'Pre-Registration' with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
